Step 1: View/Print the Full Color Brochure.

Step 2: Fill out the Program Application and Questionnaire.

To apply for the Youth On Purpose Program, please download and fill out this application, sign the privacy policy, fill out the questionnaire, and then send/email/fax back to us.

Step 3: Arrange Payment Options.

Paypal payments will be enabled 8/04/2008.
